Job Description:
- Reports to Health Facility in charge in Basic Health Center (BHC)
- Collaborates with: Members of the Basic health Centre (BHC), and the MoFA-France Project staff.
- Actively contributes to Basic Health Centre and MoFA-France Project Meeting at field level.
2. Summary of Key Functions
- Selection CHWs and establishment of the HPs
- Assist and support the trainings of CHWs.
- Help to determine the needs for CHWs training.
- Assist practice during the training and exercises.
- To make sure and re supply kits needed for each CHW.
- To guide CHWs in developing and implementing their programs.
- Assist Establishment of community health shuras in the health post and health facility levels.
- Providing on job trainings to CHWs.
- Review and evaluation of CHW’s achievements.
- To make schedule for monthly meetings (CHWs and health Shura meetings).
- Supervision of CHWs activities and all health activities in the area.
- Supervision and establishment coordination of public health programs between
community and health facility.
- Responsible to support community in finding and solving health problems.
- Assist HF staff in providing plans of community health programs in the health facility and the catchments areas.
- Conduct health education sessions according to the health facility health education plan.
- Implementing, supervision and evaluation of CHW activities in the catchment’s areas.
- Completing MAR in heath post and MAAR in the health facility.
- Integration of MAAR and assist the health facility responsible in order to make monthly activity report and usage of graphs to monitor health program in health center.
- Assist and supervise any surveys done in the area.
- To find and report any emergency cases which may interrupt the programs to head of health facility.
- Maintain coordination and support community-based volunteers’ (CHWs) activities in health facility catchment area for the referral system improvement.
- Regular reporting to health facility.
- To supervision the quality of pictorial registers and community map and help CHW to solve the problems.
- To use and report Health committee information to monitor the health status and deliver the report to head of health facility.
3. KEY RESULTS EXPECTED
- CHWs selected and HPs established in the surrounding areas of the Health Facility.
- Local community awareness level improved regarding active participation in organizing mass campaigns and vaccination program.
Community level referral of increased to Health Facility.

4.4 Languages Skills:
- Fluency in spoken and written Dari and Pashtu is a requirement. English Language skills is a plus point.
