Job Description:
Provide senior anesthesia leadership and advanced perioperative care for high-risk cardiac surgical patients (neonates, infants, children, and related adults): providing safe anesthesia for complex cardiac surgeries and catheter procedures, leading airway/ventilation and hemodynamic management, supervising trainees, and contributing to anesthesia governance, training, and service development.
Key Responsibilities
- Provide professional anesthesia for elective and emergency cardiac surgeries and high-risk catheter/hybrid procedures; determine and implement appropriate techniques based on the cardiac problem and surgical plan.
- Lead perioperative hemodynamic management: induction of anesthesia, invasive arterial/central monitoring, vasoactive/inotropic drug doses, temperature control, and coordinated post-bypass care with surgery and perfusion.
- Airway and ventilation management for complex cardiac patients of different ages: safe pediatric intubation, lung-protective ventilation strategies, management of arterial hypertension, and use of advanced ventilatory modes.
- Anesthesia and sedation management in the cath lab and hybrid room: creating shared airway plans, monitoring procedural sedation, and ensuring procedural safety.
- Perform and approve preoperative anesthesia assessment and optimization (medication/anticoagulation management, fluid status, co-morbidities) and document plans surrounding the operation.
- Prescribe and administer anesthesia, blood transfusions, and perioperative pain/sedation; ensure accurate recording of medications, fluids, and interventions and compliance with safety protocols.
- Crisis management and lead the emergency response: managing difficult airway, massive blood transfusion, cardiac arrest and activating emergency operations; ensuring safe elevation and referral.
- Develop, implement and audit anesthesia protocols, checklists and critical algorithms (e.g. extubation criteria, blood transfusion, reversal of anticoagulation); lead mortality and morbidity reviews and quality improvement initiatives.
- Supervise, teach, and assess the competency of anesthesia students and operations personnel; conduct simulations and in-service training for operational and crisis scenarios.
- Provide clinical advice for procurement and maintenance of anesthesia equipment (machines, ventilators, monitors, TEE probes); coordinate calibration/repair with biomedical engineering and maintain documentation and HMIS records surrounding operations.
Job Requirements:
Required :
- Medical degree (MBBS/MD or equivalent).
- Postgraduate (specialization) degree in anesthesia/anesthesiology (such as MD Anesthesia, FCPS Anesthesia, FRCA or equivalent).
- Current medical registration or qualification with the Afghan Medical Council (AMC); valid work permit.
- Current lifesaving certifications (ACLS and PALS/APLS for children)
Preferred :
- Fellowship or formal training/experience in cardiac anesthesia or pediatric cardiac anesthesia.
- Experience in TEE and intraoperative echo interpretation or formal TEE training.
Minimum experience
- At least 5 years of post-specialization clinical experience in anesthesia, with at least 2–3 years of significant experience in cardiac and/or pediatric cardiac anesthesia.
- Proven track record of leadership/supervisory roles, teaching and participation in clinical governance.
