About World Health Organization

The World Health Organization (WHO) is the technical and coordinating authority for health within the United Nations system. It is responsible for providing leadership in global health matters, shaping the health research agenda, setting norms and standards, articulating evidence-based policy options, providing technical and implementation support and monitoring and assessing health trends. In Afghanistan, WHO works to improve the health and well-being of all Afghans by preventing and reducing mortality, morbidity and disability and strengthening health services in close coordination with national and international partners.

Job Summary

Afghanistan’s health system is highly dependent on Household Out-of-Pocket (OOP) spending and donor support. According to the Afghanistan National Health Account (NHA) 2021, the current health expenditure in 2021 is around USD 3 billion. The highest proportion of expenditure comes from household out-of-pocket (OOP), accounting for 77.2 % of current health expenditure (CHE). The second largest health expenditure comes from donors, which is 19.3%, and the third is the government budget allocated to health which is 3.3 % of Current Health Expenditure (CHE). After the political transition in Afghanistan, the economic status changed, which affected the health sector, especially health financing, and increased reliance on donor assistance due to the low government share of the total health expenditure.

It is recognized that there is a need for an institutionalized mechanism to produce regular and reliable estimations of available financing for health, mapping both on-budget as well as off-budget expenditures. This effort is crucial to help financial planning for public health expenditures in Afghanistan, especially in a context where economic growth in the coming years does not appear to be strong enough to guarantee that domestic funding for health will be a priority, while at the same time external resource commitments for health might gradually reduce in the coming years.

Given this situation, WHO in coordination with MoPH and financial support from Gavi (the Vaccine Alliance) plans to conduct Afghanistan Sub-National Health Sector Resource Mapping 2023 exercise, including both on-budget and off-budget expenditures during 2022 as well as commitments for the next two years (2024-2025). The resource mapping exercise takes stock of available resources for 2023 and forward-looking budgets for 2024 and 2025. The findings in this report will help MoPH and its partners understand the overall resource envelope for the sector, allow more informed and efficient investments in health, and better prioritize the most needed health interventions.

Duties & Responsibilities

Take stock of health resources and their distribution and allocation at the sub-national level to better understand available and planned resource envelop for the health sector in Afghanistan and help all stakeholders to jointly align health investments based on sector priorities.

3. Overall objective:

The overall objective of the sub-national resource mapping is to support all health stakeholders to better coordinate activities as well as provide evidence for national planning and budgeting process through tracking government and development partners’ committed budget, disbursement, and spending for the health sector.

4. Resource Mapping Scope

The main scope of this resource mapping exercise is to identify on-budget and off-budget expenditures during 2023 as well as commitments for the next two years (2024-2025). In order to fulfill this objective, the scope of this resource mapping is divided into two main parts: (i) resource mapping from the donors and development partners, (ii) resource mapping by the government of Afghanistan through MOPH. The MOPH resource mapping mainly covers items and activities financed via the ordinary budget for the year 2023. The data collected through resource mapping exercise will also contribute to National Health Account (NHA) development and the NHA team will be closely involved from the start of the process. This will help the MoPH institutionalize regular resource mapping, their distribution and monitoring.

5. Work to be performed:

a) Resource mapping exercise designed and implemented in accordance to the timeline for the study

b) The consultant shall use the existing, Global Financing Facility (GFF) tool and propose a clear methodology for data collection and analysis

c) Effectively completed data collection and data analysis with focus on the scope of the study

d) Report of the study findings finalized

e) Submission of the whole documents and data set to the WHO at the end of the exercise

Deliverables:

1. Completion of design and data collection and analysis tools

2. Completion of data collection, cleaning, and analysis

3. Final Report of the study and final data set

Job Requirements

1. University degree in public health, economics, statistics

2. Minimum three years’ experience in government, NGOs, UNs in the field of : Data management and analysis, planning, financial management, accounting, public health works, or health economics

3. Proficient in use of Microsoft Excel, Word and SPSS

4. Excellent English written and verbal communication skills

5. High report writing skill is essential

Criteria/ Procedure:

 Screening of the candidates;

 Long List – The candidates shall be long listed base on the minimum qualifications and relevant experience to the Vacancy Notice (VN).

 Short List – Further shortlist shall be done base on years of relevant experience, higher education and required skills.

 Assessment method:

 WHO may conduct written/presentation and interview or combination of the mentioned mediums to select the most qualified candidate.
