Qualifications:

The Afghan Red Crescent Society, founded in 1934, is an independent and humanitarian organization recognized by the Afghan government as a charitable and impartial institution for government agencies. The Afghan Red Crescent Society (ARCS) plays a significant role in providing voluntary services and humanitarian assistance. According to the law and the statutes, the National Society conducts its activities in accordance with its seven principles: philanthropy, impartiality, independence, voluntary service, unity and inclusiveness. Among other things, the National Society is tasked with assisting affected communities, providing emergency shelter and distributing relief items to vulnerable people. Through a network of 7 regional zones and 34 provincial offices, with over 20,000 trained staff and volunteers, the ARCS works in partnership with national and international organizations at the country level to alleviate humanitarian suffering. Over the past decades, the ARCS has managed to maintain its impartiality and enjoys a reputation that enables its teams to carry out principled humanitarian actions across conflict lines, including in large parts of areas controlled by armed opposition groups. Job Description:

Provide senior consultant level transitional care for post-operative and medical pediatric cardiac patients; lead the clinical team in cardiac ICU shifts, manage advanced organ support (including ECMO if available), drive clinical governance and training, and ensure safe, timely, and family-centered transitional care in accordance with Afghan Red Crescent Society standards.

Key Responsibilities

  1. Provide senior clinical leadership for the Cardiac ICU: assessment, stabilization and management of neonates, infants and children following cardiac surgery and for medical cardiac admissions; set the unit’s daily priorities.
  2. Invasive hemodynamic and fluid management leadership: ordering and interpreting invasive monitoring (arterial/central lines), adjusting doses of vasoactive and inotropic drugs, fluid strategy management and coordination of post-bypass circulatory goals with surgery and perfusion.
  3. Respiratory Support and Weaning Management: Management of ventilator strategies for neonates and children, monitoring of advanced ventilatory modes, troubleshooting respiratory changes, and deciding on extubation readiness.
  4. Initiation and management of external body support: initiation of ECMO when available, leading bedside management and venography; coordination of cannulation and continuous care with perfusion, surgical and ICU teams.
  5. Oversee multi-disciplinary cycles, recommendations, and escalation plans: Ensure clear and documented management plans, timely escalations, and safe transfer decisions (operating room, cath lab, or other units).
  6. Leading emergency responses and surgical escalation: Management of tamponade, massive bleeding, cardiac arrest, life-threatening arrhythmias, and making decisions about revision or emergency interventions.
  7. Staff Training, Supervision and Certification: Train and evaluate students, nurses and related staff in advanced procedures (central/arterial lines, chest drains), bedside ultrasound, ECMO drills and transitional care competencies.
  8. Ensure safety of medications and devices: monitoring weight-based doses, anticoagulation protocols (post-operatively and in the ECMO setting), blood product management, maintaining device/ECMO/line logbooks and ensuring accuracy of HMIS/ECU records.
  9. Drive quality and infection prevention: Conduct mortality and morbidity reviews, root cause analyses, audits (device/line bundles, ventilator-related swabs and central line infection indicators) and implement corrective actions; ensure adherence to infection prevention and device care bundles.

Preparation and coordination for operations: Maintaining the readiness of the ICU for surgical lists (equipment, blood availability, augmentation routes), advising on the procurement and maintenance of ICU equipment, participating in transfer and extrication decisions and senior on-call responsibilities such as covering assigned duties in the rotation.

Job Requirements:

Required :

  • Medical degree (MBBS/MD or equivalent).
  • Postgraduate Diploma in Pediatric Intensivist and Pediatric Transitional Care or Pediatric Transitional Care Fellowship/Certification (or equivalent recognized qualification).
  • Current medical registration/license or registration eligibility with the Afghan Medical Council.
  • Current PALS/APLS and Neonatal Resuscitation (NRP) certifications.

Preferred :

  • Fellowship or formal training in pediatric cardiac intensive care or documented experience in a pediatric cardiac intensive care unit.
  • ECMO certification, bedside ultrasound proficiency, advanced ventilator training and neonatal/infant external body support experience.

Minimum experience

  • At least 3 years of post-specialization clinical experience in general, with at least 1–2 years of significant experience in pediatric critical care, and at least 2 years of focused experience in a pediatric cardiac intensive care unit or equivalent neonatal/infant cardiac critical care.
  • For foreign candidates: At least 5 years of senior level experience in recognized pediatric critical care units, and documented experience in ECMO/complex post-operative cardiac ECI cases is preferred.
